What You Need to Accredit for Your Particular Audience
General Information
Before beginning to plan for an accredited event, please contact CPDME at CPDME.Accreditation@dal.ca. We are happy to help clarify what is required or to answer any questions you may have.
NOTE:
- If your event involves family physicians, Dr. Michael Fleming (CPDME) must be a member of the Planning Committee.
- If you require a Simulation or Self Assessment (Section 3) package, please contact us for more information.
- If you wish to do a Level 2 or Level 3 accreditation, please contact us for more information
Specific Information
There are three different accreditation packages, depending on the target audience for the program/event. Please select your accreditation package accordingly.
- CFPC family physicians only - FP
- Royal College specialists only - SP
- Combination of both CFPC family physicians and Royal College specialists - BOTH
NOTE: These PDF documents are fillable and set up for e-signatures. Please click on the link in the appropriate column to download and save the PDF to your computer. Then use your PDF application to complete the forms. (Some of the fillable features, like the e-sigantures, may not work properly if you use your web browser to complete the forms.) If you use an APPLE computer these forms may not download or save properly.

Accreditation Policy – This differs for each group. Please read before you begin. |
