Accreditation Packages

General Information

Before you start planning an accredited/certified event, please contact CPDME at CPDME.Accreditation@dal.ca. We are happy to help clarify what is required or to answer any questions you may have.

NOTE: For our office to accredit/certify a program, the Dalhousie Faculty of Medicine must be significantly involved in the planning of the program. Therefore, at least one physician, faculty member of the Dalhousie Faculty of Medicine must be included in the planning committee.

Specific Information

The College of Family Physicians of Canada (CFPC) and the Royal College of Physicians and Surgeons of Canada (RCPSC) have different accreditation/certification categories.

NOTE: The CFPC and RCPSC are making changes to their standards/program structure. These changes are anticipated to occur in late 2024. We will do our best to keep our website and documents up to date. Please contact us if you have any questions.

Due to the changes in the CFPC's certification structure, the deadline for processing Level 2/3 applications is November 12, 2024. That means any Level 2/3 applications ought to be submitted by September 17, 2024.

Most programs fall under the Level 1/Section 1 Group Learning category, and our standard accreditation/certification package (below) has been developed accordingly. Please contact us if you require further information about the different categories and/or if you intend to apply for Level 2/3 or Section 3 credits. (We will ensure you have the correct application forms and category requirements.)

Required Forms  
Standard Application Form To be completed by the planning committee Chair.
Conflict of Interest (COI) form

Part 1 to be completed by the planning committee Chair or designate. Part 2 to be completed by all planning committee members (except administrative staff), speakers/presenters, moderators, facilitators, and authors.

Planning Committee Membership (PCM) form Part 1 to be completed by the planning committee Chair or designate. Part 2 to be completed by all physicians on the planning committee.

 

NOTE: Level 2/3 and Section 3 applications must have all the documentation for a standard application, as well as the appropriate Level 2/3 and/or Section 3 application form and supporting documentation. Please contact us CPDME.Accreditation@dal.ca for details.
