Urgent
Criteria (not all inclusive)
  • Sudden and marked decline in function
  • Post-acute stroke rehabilitation (inclusive of all subspecialty categories)
  • Post SCI
Process
1) FAX referral marked URGENT
  • Medications
  • Current x-rays
  • Recent lab results
Semi-Urgent
Criteria (not all inclusive)
  • Decline in function over weeks to months
  • Acute musculo-skeletal impairments
  • Amputation
Process
1) FAX or mail referral
  • Medications
  • Current x-rays
  • Recent lab results
Non-Urgent
Criteria (not all inclusive)
  • Assessment and management of the above subspecialty categories
