Supporting patients before and after lung transplantation
The Nova Scotia Lung Transplant Program provides assessment, coordination, and long-term follow-up care for adults living with advanced lung disease who may require a lung transplant, as well as patients returning home after receiving a lung transplant elsewhere in Canada.
Working closely with transplant centres across the country, our multidisciplinary team supports patients through every stage of the transplant journey – from referral and assessment to lifelong follow-up care after transplant.
