Immunization Information
All residents must complete this form [PDF - 76kB].
Please ensure all your immunizations are updated according to the immunizations listed.
If trainees have not received Hepatitis B shots, we suggest you start the process immediately since it could take up to 6 months to complete. Also, please ensure that your DTP includes Pertussis.
You must provide documentation/titres of your immunizations.
Dalhousie Graduates may provide their DalMedix as their supporting documentation.
